I love extreme high heels. I can walk gracefully in 7" stilettoes. I own a pair of the shoes pictured and I absolutely cannot walk in them as the goemetry of the things places my toes BEHIND my heels. They are for photo shoots only! Notice that any photo of these shoes are at a 45 degree angle. If you look at them upright, you can see they are a novelty item only. Don't get me wrong, though! I put them on and try to stand in them, but it's really quite impossible.

Eeeeek! Those are great! What brand are they? Where'd they come from? What size are they? Wanna' sell 'em?

The boots you pictured are off-the-shelf "Pony-Boots" or "Pony-Hoofs" with the only custom feature being the silver horns added at the heels. I cannot get a picture to come up with a post, but here is a link to a picture of stock pony-boots. They were created by an Australian fetish purveyer called Kaysers, but I think they've gone out of business. Google "Pony-Play" and "Pony-Boots" and you should be able to find a pair which you can add the custom feature. (Someone actually makes a pair with actual horseshoes on the bottom!)

"Perhaps if they were badgered with requests, they'd see a market for Ultra20! Eh?"

I E-mailed 6IHF a couple of times and eventually got them to reoply that the ultra 20 is not being produced, not even as a custom one-off. And no knowledge if or when production will be resumed.

I have gone to the post office and mailed keys to myself, then locked a pair of seven inch stilettos on! When the keys came 48 hrs later, I still had to wear a pair of 5" heels to get around. It took a full day before I could stand flat-footed. I LOVED IT! Buy (2) dog-collar choker-chains, pass under the heel, cross over at the back of the ankle and padlock in front. You CANNOT remove your shoes, and the chains look AWESOME! As a starting point, I have a pair of 20" chains and a pair of 22" chains depending upon the shoe.
Ballet Boots, How long can you last?

Go to an actual ballet site and purchase lamb's wool and gell inserts designed for ballet toe dancers. Makes all the difference.
